Complete and return this form only if you wish to withdraw from the contract within 14 days of receipt of the goods (Article 7 of the Terms and Conditions). You can print the form or send its text by e-mail.

Send the goods back to U.M.S., s.r.o., Studená 5, 821 04 Bratislava, no later than 14 days after withdrawal. The direct costs of returning the goods are borne by the consumer.
